MENTAL HEALTH SERVICES

Mental Health Services

Since 1976, JASA has offered geriatric mental health services in recognition that older adults have unique psychological needs. Today, JASA continues to provide and expand a variety of services to support older adults who have behavioral health issues, helping them to lead healthier and more fulfilled lives.

GERIATRIC MENTAL HEALTH OUTPATIENT TREATMENT SERVICES

JASA maintains New York State-licensed mental health outpatient treatment services in the Bronx. This clinic offers a range of treatment options for seniors suffering from mental health issues. Services include individual and group treatment, pharmacological therapy, and assistance in accessing a broad range of community-based social services. Services are provided at our clinic, at our ten satellite clinics, via telehealth, and in clients' homes as medically necessary on a limited basis.

No one will be denied access to services due to inability to pay, and there is a discounted/sliding fee schedule available based on family size and income.

LIVING IN PLACE PROGRAM

Through a contract with the NYC Department of Health and Mental Hygiene and Public Health Solutions. The Living in Place program addresses social determinants of health specific to the aging population (e.g., economic stability, reduce the risk of homelessness or placement in assisted living, engagement in social activities, access to and use of medical care, access to nutritional foods, etc.) which disproportionately impact individuals with serious mental illness (SMI), leading to an acceleration of the aging process, a decrease in the ability to age in place, and a reduction in overall life expectancy.

Person-Centered Care: Program staff maintain regular contact with members of the client’s support system and ensure that the client is living in a stable and safe environment, including ensuring that any needed medical equipment is ordered, received, and installed (if necessary).

This program does not require insurance and it is of no cost to clients.

Population Served: Older adults, ages 55 and above, with serious mental illness (SMI) to age in place in their residences.

Eligibility Criteria:

  • Must reside in the Bronx *
  • Must be 55 years or older
  • Must have a documented diagnosis of an SMI (for example: schizophrenia, bipolar disorder)
  • Currently engaged in psychiatric care
  • Demonstrating mental health stability per psychiatric provider.
  • Must have demonstrated a need for additional support for individuals to remain in their current residential setting (Letter from mental health provider to justify the need for additional support).
  • Must reside in permanent or supportive housing

Immediate Ineligibility:

  • If the individual does not demonstrate stability in the community (hx of reoccurring incarcerations, multiple psychiatric hospitalizations within 12 months, non-compliance with mental health treatment)
  • Must not reside in adult care facilities (nursing homes) or short-term transitional living residences or shelters
  • Cannot be currently enrolled in ACT (Assertive Community Treatment), FACT (Forensic Assertive Community Treatment) or IMT (Intensive Mobile Treatment)
