Biographies of Presenters
Keynote Presenter: Dr. Tara Anglim, EdD, LCSW-R, ACHP-SW, CDTLF Associate Executive Director of Culture & Experience, Caregivers Centers, Northwell Health-Peconic Bay Medical Center
Dr. Anglim is a highly accomplished Licensed Clinical Social Worker with Advanced Certification in Hospice and Palliative Care. She is dedicated to supporting individuals and families dealing with serious illnesses and disabilities. As a leader in social work and healthcare, her interdisciplinary collaborations drive innovation and unique solutions to break down barriers and inequity within traditional healthcare models. As the Associate Executive Director of Culture and Experience at Northwell Health-Peconic Bay Medical Center, Dr. Anglim has been instrumental in advocating for person and family-centered care, leading to significant achievements such as leading the hospital’s first Joint Commission Palliative Care Certification, establishing community-based education on Goals of Care, NYS MOLST, and the 4Ms framework. She established a bereavement Plan of Care for individuals experiencing loss, and in 2018, founded Long Island’s first hospital-based Caregivers Center, providing free access to support, education, and resources for family caregivers. Her leadership extends to supporting the nine Family Caregiver Centers throughout the Northwell Health system, which has aided thousands of family caregivers.
Dr. Anglim is an active voice for family caregivers, contributing to policy, research, and community outreach. Her research and work with family caregivers fuels her passion to elevate their stories and experiences to help advocate for change. She serves on various national, state, and local committees including the National Alliance for Caregiving’s Executive Steering Committee for Cancer Caregiver Collaborative, New York State eMOLST Implementation Committee where she is chairing the Patient, Family, and Caregiver Advocacy Sub-Committee, Northwell Health’s Advanced Illness Faculty, the Town of Riverhead’s Senior Advisory Council, and the Alzheimer Association’s Long Island Caregiver Conference Committee. She is an adjunct professor at Stonybrook University Graduate School of Social Welfare in the Integrated Health track. Her academic credentials include a Master of Social Work from Fordham University, and a Doctorate of Education from Molloy University, where her research examined family caregivers’ experiences within the healthcare continuum.
Presenter: Maria Miranda, LMSW, Case Management Social Worker, Presbyterian Senior Services
Maria Miranda, LMSW, has been a practicing social worker for almost 2 years. She received her Bachelor's Degree in Sociology and Master's Degree in Social Work from Fordham University. Maria currently works for Presbyterian Senior Services in their newly established Bronx Case Management Department. She previously worked as a social worker for JASA's East Bronx Case Management Department. Maria's interest and passion for providing support and services to homebound older adults stems from her lifelong experience as a youth and young adult caregiver to her grandparents, having started this journey from the age of 9 to the age of 24. Maria's caregiving experience has also fueled her newly found passion for advocacy work, participating as a National Advisory Council Member for the American Association of Caregiving Youth along with advocacy ties with the National Alliance for Caregiving, Caring Across Generations (part of the 2024 Caregiver Fellow Cohort) and NY Caring Majority. In sharing her caregiving story, Maria seeks to inspire, encourage, and empower other caregivers to do the same to bring about creation and change in U.S. caregiver policy.
Presenter: Tom Chiodo, Executive Producer, Caregiving and Special Projects at WETA PBS
Tom Chiodo is the Executive Producer of Special Projects at WETA, the flagship PBS station in Washington, DC. Notable documentaries include Ken Burns’ "Hiding in Plain Sight: Youth Mental Illness," the Emmy-nominated "The Gene: An Intimate History," and the DuPont Columbia Award & Emmy-nominated "Cancer: Emperor of All Maladies." Currently serving as Executive Producer for upcoming projects such as "Caregiving" (June 2025), "Remote Chance: Health Care in Rural America," "Defeating Dementia," and "Climate Kitchen." Previously held senior positions at the Entertainment Industry Foundation, Rubenstein Associates, U.S. Department of Health and Human Services, and Massachusetts Department of Public Health. Co-authored “Home Care for Respirator Dependent Children" in the New England Journal of Medicine. Contributing writer for Forbes.com.
