JASA is seeking a licensed clinician to join our team delivering mental health services to older New Yorkers in the Bronx.

The Clinical Social Worker, Home Visits will:

Provide evidence-based mental health interventions to homebound individuals and families in their homes
Administer evidence-based assessments and screening tools such as PHQ-9, GAD-7, and Mental Status Examinations to older clients to screen for possible disorders and impairments requiring mental health services
· Develop, implement, monitor and document treatment plans

· Provide psychoeducation to clients and collaterals including appropriate information regarding “typical” physical, social and psychological human development, challenges to functioning presented by conditions of impairment, disease, social stresses and dysfunction and gero-psychiatric disorders, and ways of coping and preserving individual functioning and autonomy

· As appropriate, make referrals to, and maintain cooperative relationships with public and voluntary agencies, programs, services and facilities; provide information to clients and collaterals about health, welfare, education and recreation services available to serve the client

Participate in regular supervision as well as training and unit meetings as needed
Participate in quality improvement efforts by reporting problems observed in a timely manner and making suggestions to supervisors regularly of ways to improve quality
Complete required electronic case records, reports, and statistics within mandated timeframes
Perform such other duties as assigned


The Clinical Social Worker, Home Visits will have the following qualifications:

New York State License in Clinical Social Work or Clinical Psychology
Ability to read, write, speak, and understand English; Spanish language skills preferred
Good writing/record keeping skills
Basic computer skills; familiarity with EHR preferred
Ability to travel using mass transit and to negotiate any steps and barriers in clients' residences
An understanding and appreciation of the roles of race, religion, ethnicity, sexual preference, and individual values have as they relate to serving clients and families
