General Responsibilities:

The JASA NY Connects Brooklyn Specialist will use a person-centered planning approach to provide individuals with Information and Assistance including public benefit programs such as Medicaid. The worker will provide Long Term Care Options Counseling; assist individuals and/or their families and caregivers in making decisions regarding current or future LTSS needs as appropriate both in a field and office setting. The NY Connects Specialist will perform outreach and education for groups to increase public awareness of the NY Connects program as assigned by the Director of NY Connects.

Specific Duties and Responsibilities

• Provide objective and unbiased information and assistance on all long term care services/supports; provide follow-up with individuals regarding referrals to services made.

• Provide appropriate information and linkages to programs and/or services that may be explored to meet identified needs.

• Conduct the No Wrong Door (NWD) screen with individuals over the phone or in-person, in order to obtain preliminary information about LTSS needs and possible programmatic and/or financial eligibility for various programs including Medicaid.

• Assist individuals, caregivers, and or/family members in completing the online questionnaire, as requested, and consult summary report results from completed online questionnaires when conducting NWD screen.

• Serve as Long Term Care Options Counselor, building a helping relationship with individuals and/or their families to provide options counseling through a person-centered interview.

• Assist Care Transitions Specialist in implementing and revising care transitions plan and providing follow-up with client and/or family as needed.

• As a direct provider of information and assistance, comply with all applicable New York State NY Connects and JASA Standards when interacting with all contacts to NY Connects and performing all other job functions.

• Serve as an available resource to individuals, caregivers and families to assist in transitions from acute care or skilled nursing facilities to home or least restrictive settings

• Perform outreach and education activities with groups to increase awareness of the program, its core functions, its objectives and value to the community.

• Participate in professional trainings to acquire and update skills (i.e. attend in-services and NYSOFA/DOH/DFTA sponsored meetings and trainings.

• Complete required documentation, case records, reports, and statistics utilizing computers and databases as required.

• In all responsibilities listed above, utilize a strength based, person centered approach to support the individual’s independence and self-determination while working with a diverse population and respond to all individuals in a professional, non-judgmental and culturally appropriate manner.

• Work under supervision of JASA’s Director NY Connects or designee.


• A Bachelor’s Degree from an accredited college in Social Work, Psychology, Counseling, Gerontology

Desired Skills and Specifications:

• Strong understanding and knowledge of the long-term care support services systems for people of all ages with disabilities in New York City.

• Familiarity with the older adult and people with disabilities populations.

• Compassionate, culturally sensitive with excellent listening skills.

• Experience with screening and interviewing individuals.

• Capacities to problem - solve and think critically.

• Strong verbal and written communication skills, including documentation.

• Experience in public speaking.

• Proficiency in computer literacy and web-based systems.

• Bi-lingual (plus).
