Community Guardian Program
JASA assumes the legal role of community guardian for adults (18 and over) who have been judged legally incapacitated by the courts and require a legal guardian. Through assessment, comprehensive service planning, regular visits, ongoing support and regular reports to the courts, JASA ensures that the needs of these individuals are met. Guardianship responsibilities can encompass a wide range of duties. Bases on court decision this can include assistance with aspects of daily living and advocacy with medical and social service providers.
Key Responsibilities
• Screen and assess each client for benefits, including Medicaid, SNAP, PA, HEAP, etc.
• Create applications and file for Medicaid, SNAP and Public Assistance on behalf of clients.
• File applications at appropriate HRA offices, and elsewhere.
• Maintain HMO and all MLTCs for the clients, including corresponding with the agencies.
• Maintain tracking database for Medicaid and SNAP/PA databases.
• Work with coworkers and other agencies to facilitate applications.
• Order all documents and follow up with financial institutions.
• File fair hearing and appeals, and attend if necessary.
• Educate and train coworkers about benefits, entitlements, Medicaid, etc.
• Experience necessary, experience in long term care preferred.
• Coordinate with nursing homes to apply for Nursing Home Medicaid benefits.
• Screen patients being admitted for a LTC stay and assess whether they will meet the financial criteria for Medicaid eligibility.
• Determine estimated NAMI and ensure income is being turned over to the facility based on Medicaid guidelines (for pending cases) or as determined by DDS (for approved cases). This includes coordinating with Social Security, pensions, and I-TC insurance companies to have income redirected to the facility when appropriate.
• Coordinate with MLTCs, MMCs and with DDS regarding Medicaid conversions and recertification to ensure all Medicaid and MLTC patients have appropriate coverage.
Qualifications:
• BA/BS or prior experience working in Medicaid Services Coordination and/ or public benefits coordination or similar position.
• A basic knowledge and experience with Medicaid law, including but not limited to:
o Income and resource guidelines.
o Citizenship and residency requirements.
o Allowable and non- allowable asset transfers.
o Familiarity with guardianships and Power of Attorney laws as they pertain to the Medicaid application process.
• An understanding of the forms and notices used by HRA and the other Departments of Social Services in NY State.
• Calm demeanor, especially when dealing with emergencies, deadlines or other high- stress circumstances.
• Computer literate. Able to use Microsoft and Google products. Able to learn a new case management system.
Compensation: Salary DOE with competitive benefit package.
