General Responsibilities:


Operates vehicle to transport clients to and from program location.  Assists clients in and out of vehicles.  Maintains vehicles.  Provides supportive client services under the direction of the director.



Specific Duties and Responsibilities (DRIVER):


  • Drives assigned vehicle.
  • Arranges for purchase of gas and oil for vehicle when needed and in accordance with agency procedures, check fluids, tires as appropriate.  Ensure that vehicle is equipped with spare tire, jack and other necessary emergency equipment, maintains lift gate in working order.
  • Immediately reports any accident or traffic violations to supervisor.
  • Obeys all traffic regulations.
  • Maintains vehicle in a clean condition at all times.
  • Maintains trip tickets and any other records that may be required.
  • Escorts and assists and protects elderly and infirm clients in moving from home to the vehicle and returning back to the apartment.
  • Immediately alerts supervisor to any unusual situation regarding the client e.g. person doesn't answer the door; person appears to be ill; person's behavior is different than it usually is.  When in doubt regarding any situation, immediately telephones the supervisor.
  • Performs other related tasks as requested or assigned.







Specific Duties and Responsibilities (MENTAL HEALTH AIDE):


  • Encourages socialization by introducing activities to reduce isolation including conversation and companionship.
  • Observes clients’ appearance, mental status, behavior, mood, etc., to note any changes.
  • Assists social work staff in teaching life skills such as; shopping, cooking, laundry, dusting and mopping,
  • Escorts clients on trips to banks, medical/dental appointments, senior center, etc.
  • Maintains records in a current and legible manner as required by the program, as specified in the program manual, and as mandated by the funding source.
  • Keeps a statistical record of each visit made; date, number of hours worked, etc.
  • Records reasons for all cancelled or missed appointments.
  • Reports on activities and observations at staff meetings.
  • Attends meetings and trainings as required.
  • Performs other related tasks as requested or assigned.





  • Three years driving experience and a valid N.Y. State CDL driver’s license with no moving violations.
  • No driving while intoxicated convictions.
  • Ability to carry groceries, lift heavy objects, climb stairs and stand for long hours.
  • Ability to read, write and speak English clearly and effectively.  Must be able to complete forms and other paperwork in English.
  • Willingness to work with mentally impaired older adults including those with aberrant behaviors and unconventional lifestyles.
  • Absence of communicable diseases such as might be transmitted through airborne contact or food handling.
