Services - Support & Assistance - Case Management

Social Work Staff will assess the needs of the referred client and will identify services and resources that will enable the individual to remain in the community. Case management includes referring and arranging services as well as monitoring and coordinating programs while providing supportive counseling to an individual, couple or family.

Turn to Case Management for:

  • Assistance with light housekeeping: cleaning, shopping, laundry
  • Help with bathing, washing up and dressing
  • Modest cost share based upon income
