JASA and Oxford Medicare HMO have formed a partnership to provide at-risk seniors enrolled with Oxford easy access to social services available in their community. The goal is to allow these seniors to live independently with dignity.

The fee-for-service initiative:

• Assigns a social worker
• Assesses individual needs
• Creates an individual plan
• Links to Meals on Wheels, food stamps, transportation
• Arranges for home visits
