Taking charge of an aging, frail relative is a challenging task. When you require the services of a quality hands-on caregiver, JASACare will step in as your partner. A Geriatric Care Manager (GCM) coordinates every aspect of services, beginning with a home visit to assess your relative's individual needs. The GCM assigns a home health aide, and works with a personal physician and a Registered Nurse to oversee medical care. The GCM provides referrals to other experts, and stays in touch with family members. Services are billed by the hour to the senior citizen or to a family member. Referrals come directly from the senior's family or a friend, hospital discharge planners, or the personal physician.

The Geriatric Care Manager will:

• Supervise the home health aide's priorities: housekeeping and meals
• Coordinate medical care and visits to physicians, monitor medications
• Locate proper housing
• Arrange social activities
• Provide referrals to legal and financial guardians
• Assist with entitlement applications
• Communicate with family members
