JOB TITLE: Service Manager

REPORTS TO: Assistant Director of Home Care

SUMMARY:
To oversee and manage the Service Coordinators and related field personnel. This includes ensuring the quality and quantity of service provided is consistent with agency mission, policy, and goals. In addition, to ensure that service and personnel are compliant with regulatory and contractual requirements. In the absence of the Assistant Director, will have management responsibility of the service provision team in conjunction with Director of Patient Services.

ESSENTIAL DUTIES AND RESPONSIBILITIES

• Oversees the day to day activities of the Service Coordinators and their assigned home care workers. Ensures that all staff works cooperatively in implementing agency policies and procedures.
• Evaluates the implementation of agency policy and procedure by staff and intervenes where necessary to ensure that quality services are provided to both contracts and private pay patients.
• Ensures that all staff are knowledgeable of contractual, regulatory, and accreditation requirements. Implements monitoring systems to evaluate and maintain staff compliance.
• Facilitates growth and development of assigned staff and acts as a resource for them.
• Provides orientation and training for all assigned staff.
• Conducts Conflict of Hours investigations and resolutions.
• Collaborates with the HR Department in developing and implementing effective staffing patterns and identifying needs.
• Monitors employment activity and utilization of home care workers, and those of the Service Coordinators. Assures maximum utilization of the work force using available automated reports, and conducting audits as necessary. Maintains personnel statistics to accurately reflect this activity.
• Participates with labor relation meetings and hearings as necessary.
• Participates in developing, coordinating, and evaluating communication and work-flow procedures.
• Participates in the development and revision of program policies and procedures.
• Participates with the agency’s ongoing Quality Improvement activities. This includes documentation and investigation of incidents, accidents, and complaints. Serves on Quality Improvement committee and/or team projects as necessary.
• In collaboration with the Assistant Director, assures after hours operation and on-call services function effectively in accordance with agency policy and procedure. This includes developing an effective and equitable on call schedule for assigned staff, and accepting on-call responsibility in time of operational need.
• In the absence of the Assistant Director, assumes management responsibility in conjunction with the Director of Patient Services, for the service management team.
• Other responsibilities as required.

WORKING CONDITIONS:
• Sitting for long periods of time, some standing and stooping. Occasional stair climbing affiliated with work environment, e.g., field visits.

• Office setting with occasional home visits. May be exposed to client elements and illnesses as well a wide variety of circumstances and conditions within the home setting. Requirement to work after hours, weekends, and holidays as needed.

JOB QUALIFICATIONS:
• Ability to effectively communicate in English, both orally and in writing.
• Additional languages helpful, particularly Spanish, Russian and/or Creole.
• Effective organizational and time management skills.
• Effective customer service skills
• Knowledge of New York State Dept. of Health regulations for Licensed Home Care Service Agencies.
• College Degree preferred or equivalent experience.
• Two years experience in supervisory capacity at a home health agency (i.e., CHHA, LTHHCP, LHCSA).
• Must be computer literate, including ability to learn and use program-specific software. Knowledge of current version of Microsoft Office and Google Suite. Experience with automated time-leave systems preferred
• Experience working with New York State Department of Health regulations for Licensed Home Care Service Agencies required.
