SUMMARY: Provides nutritional therapy by creating nutritional programs based on the health needs of clients. Counsels clients on how to lead a healthier lifestyle.


DUTIES AND RESPONSIBILITIES:
• Assesses clients’ nutritional status
• Counsels clients on healthy eating habits and therapeutic diet
• Develops meal plans, taking both cost and clients’ preferences into account
• Evaluates the effects of meal plans and modifies the plans as needed
• Completes all required documentation
• Supervises and directs senior aide in the client’s dietary plan of care
• Provides instruction on nutritional meal plan to senior aides and home health aides
• Consults with interdisciplinary team on nutritional plans for the clients
• Keeps up with the latest nutritional science research
• Participates in agency and community programs
• Participates in agency’s in-service education programs
• Respects client and employee rights and privacy, ensures security of protected information, practices in an ethical manner and adheres to the agency’s compliance and privacy policies
• Interprets philosophy and objectives of the agency to clients, families and others.
• Participates in the agency’s Quality Improvement Program
• Complies with agency policies and procedures; provides services in accordance with federal, state, and local laws
• Performs other job related duties as may be required or directed by the supervisor


SPECIALIZED SKILLS AND COMPETENCIES
• Demonstrates knowledge and expertise within the scope of practice of a Nutritionist
• Excellent communication skills
• Excellent interpersonal skills
• Excellent organizational ability
• Must be computer literate, including ability to learn and use program-specific software
• Ability to direct home care workers


PHYSICAL REQUIREMENTS:
• Will spend varied percentage of working time indoors or outdoors depending on program and/or client needs.
• Will encounter prolonged standing and walking.
• Will occasionally climb stairs.


WORKING CONDITIONS:
• Office setting and regular home visits, including attendance at meetings.
• Considerable exposure to stress.
• Use of auto or public transportation.
• May be exposed to client elements and illnesses as well as a wide variety of circumstances and conditions within the home setting.
• Evening and weekend work required as necessary to attend meetings, meet deadlines, and/or as part of on-call requirements.


QUALIFICATIONS:

• Bachelor’s Degree required - Nutrition

• Must be computer literate, including ability to learn and use program-specific software.
• Minimum of 2 years of professional experience preferred.
