General Responsibilities
The nurse will integrate expert health case management services for Long Island Network for Dignity and Support clients in collaboration with the program’s other staff.
Primary Responsibilities
• Promote maximum level of wellbeing among clients, assisting them to develop a management plan to take an active role in their healthcare
• Consult with staff regarding medical issues and goals of care by meeting on a regular basis and providing joint visits with program clients/patients
• Conduct in-home nursing assessments of clients with health concerns to identify risks and coordinate their health and long-term care with a focus on such issues as: polypharmacology, nutrition, vision and cognitive decline, depression, home safety, symptoms, treatment options, pain and symptom management, and medical equipment
• Provide treatment as needed and appropriate (as long prescribed by physician or NP) such as wound care and pain and symptom management
• Discuss preparation of health care proxies, MOLSTs, and other advanced directives
• Consult with current community and hospital care team and advocate for additional or alternative treatment as needed
• Ensure that individuals are not discharged from hospitals without appropriate service coordination
• Use knowledge of a range of complementary approaches to pain and symptom management through direct application or referral
• Address issues of anticipatory grief with the client/patient and family and refer as needed in collaboration with the Long Island Network for Dignity and Support
• Provide community education programs to both general community and professionals
• Participate in evaluation activities and provide qualitative and quantitative information as needed for the project
Qualifications
• Master’s degree in nursing preferred; Current NYS Registered Nurse License; ACLS/BCLS Certification
• 3-5 years of experience in Geriatrics and/or Palliative Care preferred
• Proven experience in training staff, other professionals and general community
• Desire and ability to work collaboratively and thoughtfully as part of a committed, integrated team, and build strong relationships
• Excellent interpersonal skills, including the ability to communicate effectively and concisely (both in writing and orally)
• Strong attention to detail with the ability to solve problems, prioritize work, and work independently
• Professional maturity, good judgment and ability to motivate others in a similar way
• Strong commitment to JASA’s mission
• Basic computer skills; familiarity with EHR preferred
• Ability to drive to visit clients and other partners as well as take mass transportation
• An understanding and appreciation of the roles of race, religion, ethnicity, sexual preference, and individual values have as they relate to serving clients
